Service Connected Compensation and Pension Use 21-526 series form Non-Service Connected Claim, use 21-527 series form.   Is it an Application for Burial Benefits? Use form 21-530. Is it a Dependency & Indemnity Compensation (DIC) Claim? Use 21-534. VSO: Obtain death certificate from surviving family member. If the Veteran needs assistance from a 3rd party, fill out 21-0845.
