dsf                 LEA requests evaluation of a student.         YES Permission to Evaluate consent form is sent to parents.         YES If parents agree to evaluation, the school psychologist will evaluate the child within 60 days.         If the child is eligible for services, an Evaluation Report is made and an IEP meeting is held within 30 days of the evaluation.         The IEP must be put into action within 10 school days of the IEP being completed.         The IEP must be reviewed at least once a year.         Children must be reevaluated every 3 years / reevaluated every 2 years if Intelectually Disabled.
