Help IEP teams determine whether a student needs Additional Paraprofessional Support.                     A REFERRAL IS MADE         The person making the referral fills out the Request.         The case manager provides parents with Prior Written Notice that the IEP team will meet to consider the student's need for Additional Paraprofessional Support.         AN IEP MEETING IS HELD.         The team reviews existing information and fills out Form B-1.         THE IEP TEAM MAKES A DETERMINATION OF NEED. NO The student does not require Additional Paraprofessional Support.   YES       PRESENT LEVELS OF PERFORMANCE: document the justification of why the team made their determination. SUPPLEMENTAL AIDS AND SERVICES:indicate the need for supports the team has determined the student needs.
