Student returns to or enrolls at home school or last school attended after placement in a mental health facility for 30+ consecutive school days
 
 









 
General education student
EC student (not self-contained)
EC student (self-contained or public-separate)
 









 
The intervention team convenes ASAP and reviews all available information to determine how best to support the student
An interdisciplinary team including EC Case Manager convenes ASAP to review all information and determine how best to support the student
The EC Compliance Facilitator contacts EC Placement Coordinator at 343-6960
 









 
Meetings and decisions are documented on the Intervention Team Referral Form and in Web-IT
Interdisciplinary team documents the meeting
The EC Placement Coordinator will identify appropriate potential self-contained classrooms or public separate school assignments
 









 
If student needs more intensive supports then the school follows the standard IT process and documents on the RE1
If changes to the IEP are needed then an IEP meeting will need to occur
